To order a hard copy of a LHCCREMS publication
please complete and submit the form below.
Instructions: 1. Type in the number of copies you wish to
order in the"Quantity" box.
2. Click on the corresponding "equals"
button to calculate.
3. Once you have indicated which items you wish
to order, click on the "Subtotal" button
(this will add $5.00 postage and handling).
4. Click on the "GST" button to calculate
GST.
5. Click on the "Total" button to calculate
the full cost of your order.
6. Fill in your delivery details in the area provided
at the bottom of the page.
7. Click on the "Send" button.
An invoice will be issued for each order and
publications forwarded on receipt of payment.
Please contact us if you require the publication
urgently. Alternatively, many of the publications
can be downloaded without charge from the form
below. A concessional rate is available for some
